WCPAP Application Coverage ID Lookup

To begin your WCPAP application you must select a valid Coverage ID using the Lookup search below.
  • Enter the employer's name
  • Entering the zip code is optional. The zip code may help to narrow the search results.
  • If you do not see the employer's name in the search results, try a new search by entering only the first 3 characters of the employer's name to expand the search.
  • Select the coverage ID from the search results screen to populate the WCPAP Application name and address field.
